Tendinosis is a degenerative breakdown of tendon tissue caused by repeated overload rather than active inflammation. It affects tendons such as the Achilles, patellar, rotator cuff and elbow extensors, and is common in athletes and manual workers. Progressive loading exercise is the primary treatment. What Is Tendinosis?

Tendinosis is a chronic, degenerative condition of a tendon, the tough cord of collagen that anchors muscle to bone. Under a microscope, a tendon with tendinosis shows disorganized collagen fibers, increased ground substance, small tears and new blood vessels and nerve endings growing into tissue that is normally almost bloodless. What it does not show is a large population of inflammatory cells. That is the key difference from tendinitis, which is a short-lived inflammatory reaction. Most tendon pain that has lasted longer than six to eight weeks is tendinosis, not tendinitis, which is why anti-inflammatory strategies alone often disappoint.

The tendons most often affected are the Achilles at the back of the ankle, the patellar tendon below the kneecap, the common extensor tendon at the outer elbow, the rotator cuff tendons at the shoulder, and the gluteal tendons at the hip. Tendon disorders account for a large share of musculoskeletal visits, and Achilles and elbow tendinosis are especially common between ages 30 and 60, when tendon tissue heals more slowly than it once did.

What Causes Tendinosis?

Tendinosis develops when the load placed on a tendon repeatedly exceeds its capacity to adapt. Tendons remodel slowly, over months rather than days, so a sudden jump in training volume, a new job task or a return to sport after time off can outpace the tissue. Each overload episode leaves microscopic damage. When there is not enough recovery time between episodes, the damage accumulates and the tendon’s internal structure begins to change. Age, hormonal factors, metabolic health and certain medications also influence how well tendon tissue repairs itself, which is why two people doing the same activity can have very different outcomes.

Common causes and risk factors include:

  • Rapid increases in training volume, intensity, hill work or plyometrics
  • Repetitive gripping, lifting, keyboarding or overhead work
  • Poor movement mechanics that concentrate load on one tendon
  • Prior tendon injury or an incompletely rehabilitated strain
  • Age over 30, when tendon collagen turnover slows
  • Diabetes, obesity, high cholesterol and thyroid disorders
  • Fluoroquinolone antibiotics and repeated corticosteroid injections near the tendon

What Are the Symptoms of Tendinosis?

Tendinosis pain is usually localized, predictable and load-dependent. People describe stiffness and soreness at the start of activity that eases as they warm up, then returns worse a few hours later or the next morning. The tendon is often tender in one specific spot, and it may feel thickened or nodular compared with the other side. Unlike an acute strain, there is rarely dramatic swelling, bruising or redness.

Typical symptoms include:

  • Pain that builds gradually over weeks or months
  • Morning stiffness in the tendon that loosens after a few minutes
  • Tenderness when you press directly on the tendon
  • Visible thickening or a palpable nodule along the tendon
  • Weakness or loss of push-off, grip or overhead power
  • Pain that worsens 24 hours after a hard session

Seek prompt medical attention if you feel a sudden pop with immediate weakness, cannot bear weight or push off, notice a visible gap in the tendon, or develop fever, spreading redness and heat over the area.

How Is Tendinosis Diagnosed?

Tendinosis is usually diagnosed clinically. Your physical therapist takes a history of how the pain behaves with load, then palpates the tendon to find the tender segment and checks for thickening. Load tests are the most useful part of the exam: single-leg heel raises, decline squats, resisted wrist extension or resisted grip reproduce the familiar pain in a graded way and give a baseline to measure against. Range of motion, strength and movement quality at nearby joints are also assessed, because stiffness or weakness up or down the chain often drives the overload.

Imaging is not required for most cases. Ultrasound or MRI is reserved for suspected partial or full tears, symptoms that do not respond to a well-run loading program, or when surgical planning is being considered.

How Physical Therapy Treats Tendinosis

Physical therapy is the primary evidence-based treatment for tendinosis, and the active ingredient is progressive mechanical loading. Loading the tendon in a controlled, graded way stimulates collagen synthesis and helps the tissue reorganize and tolerate force again. Rest alone tends to make tendons weaker and more sensitive, so SportsMed therapists rarely prescribe complete rest. Instead they find the level of load your tendon can handle today and build from there.

Care usually begins with pain-settling isometric holds and activity modification, then moves into heavy slow resistance and eccentric work, and finally into speed, spring and sport-specific loading. Manual therapy, dry-needling-style soft tissue work, taping and bracing are used as helpers, not as the main event. Upper limb tendinosis at the elbow, wrist and hand is often co-managed through hand and occupational therapy, where grip strengthening and workstation changes matter. Athletes finish through a structured return to sport program, and lower limb cases may use Alter-G anti-gravity treadmill training to restore running load safely. Job-related cases can be managed alongside workplace injury rehabilitation.

How Long Does Recovery Take?

Tendon tissue adapts slowly. Most people notice meaningful pain reduction within four to six weeks of consistent loading, but full recovery of tendon capacity commonly takes three to six months, and longer for tendons that have been painful for over a year. Progress is faster when you attend regularly, do the home program at the prescribed load, and keep total activity within tolerance. Progress slows with repeated flare-ups from doing too much too soon, long gaps between sessions, uncontrolled diabetes or thyroid disease, smoking, and repeated corticosteroid injections. Setbacks are normal and do not mean the program has failed.

Q: What is the difference between tendinosis and tendinitis?

A: Tendinitis is a short-term inflammatory reaction in a tendon, usually lasting days to a few weeks. Tendinosis is a longer-term degenerative change in the tendon’s collagen structure with little inflammation. Because the tissue problem is different, tendinosis responds better to progressive loading exercise than to anti-inflammatory medication alone.

A: Tendinosis rarely resolves fully with rest alone, because tendons lose capacity when they are unloaded. Symptoms may quiet down temporarily, then return when activity resumes. A structured loading program is what rebuilds tendon tolerance, and most people need eight to sixteen weeks of consistent work to see durable change.

A: Usually not. Complete rest tends to weaken the tendon further. Most people do better modifying load, reducing the aggravating activity, and continuing with tolerable strengthening. A physical therapist can set an acceptable pain level during and after exercise so you keep training without driving the tendon backward.

A: Corticosteroid injections can reduce pain for several weeks, but studies show worse long-term outcomes and higher recurrence compared with exercise-based care, and repeated injections may weaken tendon tissue. Discuss the tradeoffs with your physician. Injections are best considered as short-term relief alongside, not instead of, a loading program.
